Profile Overview

The patient portal profile includes four sections:

• Patient Information – enter detailed patient contact information here
• Insurance – enter information for up to three insurance carriers
• Responsible Party – enter Responsible Party (patient’s legal guardian) information here
• My Preferences – setup contact methods, security questions, and password settings

Forms will contain required fields that are setup by your healthcare provider.
